Health Assessments for 75 Years and Older Patients

WHAT IS A SENIOR HEALTH ASSESSMENT?
A Senior Health Assessment is a free (Medicare bulk billed) comprehensive assessment of your health, physical, psychological and social function. It helps to assist in the early identification of care needs.

WHO IS ELIGIBLE TO RECEIVE A HEALTH ASSESSMENT?
You are eligible if you are aged 75 years or over, living in a community setting. You must be a regular patient of the surgery.

WHAT HAPPENS DURING A HEALTH ASSESSMENT?
You will spend 45 minutes with the Practice Nurse who will perform some simple tests and ask you some questions about your medical history.

This appointment usually takes 45 minutes with the Practice Nurse followed by a 15 minute appointment with your GP.

This might include the following:

  • Vision check
  • Hearing check
  • Skin check
  • Urine sample
  • Blood Pressure
  • Medication review
  • Immunisation status
  • Weight
  • Height
  • Questions about your nutrition/diet
  • Questions about your social activities
  • Questions about your daily activities

  

CAN A CARER OR A FRIEND BE WITH YOU DURING THE ASSESSMENT?
A family member, carer or friend can attend the health assessment with you if you think it will help you to feel more comfortable about the assessment.
