GP Management Plans/ Team Care Arrangements

What is a GP Management Plan?

It is a written plan for the management of your disease/illness development by your GP and practice nurse in consultation with you

Why should I have one?

It helps you and your GP to better understand your condition and co-ordinate your care more effectively, particularly when you have a number of doctors and other health professionals managing your condition

Am I eligible?

Do you have a chronic condition (a medical condition present for longer than 6 months)

Some examples of a chronic condition include:

  • Diabetes
  • Asthma
  • Arthritis
  • Cancer
  • Heart Disease
  • Osteoporosis

These are only a few examples, so check with your doctor if you think you are eligible

What happens if I’m eligible

  • You can make an appointment with the practice nurse and GP. You need to allow approximately 45-60 mins for the assessment
  • Then you and your doctor/practice nurse will identify your health priorities
  • Your goals will be included in your plan
  • When preparing the plan your GP and nurse will consider your physical, social and psychological needs
  • A list of actions will be decided to help you manage your condition
  • Other health care professionals and services will be allocated if you need them. This is known as Team Care Arrangements (TCA)

What does TCA mean?

TCA stands for Team Care Arrangement. If your doctor determines you would benefit from involving other health professionals in your care then you can have up to 5 visits per calendar year to see an allied health professional

These services can include:

  • Podiatry
  • Physiotherapist
  • Dietician
  • Speech pathologist
  • Chiropractor
  • Occupational therapist
  • Audiologist
  • Osteopath
  • Physiologist

These visits are medicare rebated, check with your health professional for any out of pocket expenses
